If you've won a cash prize on KiiM-FM, here's some new important information.

Our company now requires all cash winners to fill out and return an IRS W-9 form and sign a contest winner release before a check will be issued. You can download a W-9 form and the contest release here. You can also pick it up at our studios, 575 W Roger Road, during business hours, if that's more convenient.

Once it's completed, you can FAX it to our business office at 520-887-2800, send it to us via regular mail (575 W Roger Rd, Tucson AZ 85705), scan it and email it to Tucson.prizes@cumulus.com , or drop it by in-person during business hours.

From that point, we send everything to our company's corporate office. They write the check and mail it directly to you. It might take two weeks or so from the time that you win, to the time you get your check.
